Certain activities may require authorization from the USC Institutional Official (IO) / Vice Provost for Research Advancement (VPRA). Prominent examples are Certificate of Confidentiality requests, Genome-Wide Association Studies (GWAS) Data Submissions, IRB Authorization Agreements or Unaffiliated Investigator Agreements. Following are a list of Supporting Documents to be included with request, a template of the IRB memo requesting signature and a flowchart of the signature request process. Once the IO/VPRA signature is ready, the IO/VPRA office will email the signed documents back to the IRB, PI and OPRS.
