Downloadable Petition Form for students who cannot use the online process:

  • Submit a paper petition found here for each course,
  • Provide the complete syllabus you received from your instructor on the first day of class (not a generic course outline) along with any supporting documents (see Documents that Support Your Petition). These may be paper copies, or you may email them to uscartic@usc.edu, referencing your petition.
  • Deliver or mail the petition and supporting documents to:
