Forms and Instructions

• The 201 Curriculum forms for programs, cross-list a course form, the 401 form for minors and the 501 form for off-campus programs are Microsoft Word forms designed for you to save and fill out on your computer. The 301 forms for courses are web-based.
  
  
• All forms must be sent electronically (via email) to the Curriculum Office. "Signatures" will be provided via email. Contact the Curriculum Office for assistance.
