Request for Reissue of Diploma
The University of Southern California issues only one diploma for each degree. A diploma may be reissued only under certain circumstances. The name on the diploma must reflect the legal name of the student at the time of attendance and graduation (or formal separation)from USC. The university does NOT reissue diplomas in a new name if the name change legally occurred after the student graduated There is a $100 fee to reissue a diploma.
Complete and sign the Application for Diploma Reissue Form. Please include a check or money order payable to the University of Southern California, in the amount of $100.00. The university has the right to reject a request for Diploma Reissue if the alumni has an activity restriction on his/her record. The Degree Progress Department mails reissued diplomas within 4 weeks of receipt of the completed application. Inquiries regarding records prior to 1980 require additional time for research as these records are maintained on microfilm.
