Requests by Mail
In letter form, please submit the following information, legibly printed or typed:
- Name(s) you used while attending USC
- ID number while attending USC
- Birth Date
- USC attendance dates
- All USC degrees earned
- Number of copies you are requesting
- Where you want the verification sent
- Your signature
- Your daytime telephone number
- Payment

Methods of payment are: personal check (made payable to the University of Southern California), U.S. money order, USCard, and the following credit cards; VISA, MasterCard, and Discover. Cash is not accepted.
Verification fees
Official academic record verifications are $5.00 per document.
There is no charge for verifications requested for Financial Aid related compliance.
Special handling services
Overnight Express is $18.00 per destination, plus verification fee.
You may request this service for overnight delivery within the continental United States only.
Express Mail is $40.00 per destination, plus verification fee.
You may request this service for express delivery outside the continental United States only.
