Making Changes to Your Benefit Programs (continued)

Registration of a Domestic Partnership
Medical and Dental Coverage
You must initiate the eTrac transaction within 30 days of the registration to add your registered domestic partner to your medical and dental coverage. This is a two-step process. You must first add your domestic partner as your Personal Associate and then go to the Benefit Enrollment page to enroll him or her in your plan. Coverage is effective the first of the month following approval of your eTrac transaction. If you do not enroll your domestic partner in your plan within 30 days of registration, your next opportunity will be during the university's open enrollment with coverage effective January 1.
To enroll your domestic partner, you must submit documentation acceptable to the university that substantiates the registration of your partnership. Such documentation generally consists of a state-approved certificate of registered domestic partnership but may include other documentation that USC, in its discretion, finds acceptable. Forms and instructions for registering a domestic partnership are available from the State of California at www.sos.ca.gov/dpregistry. If you fail to provide acceptable documentation of domestic partnership, USC may take such action as it deems appropriate including refusing to enroll the individual in question or terminating the enrollment of that individual.
Dental coverage is not included in the medical plan; you will need to initiate an eTrac enrollment to elect dental coverage along with medical coverage.
