Termination or Dissolution of a Domestic Partnership

Medical and Dental Coverage

You must initiate the eTrac transaction within 30 days of the effective date of the termination or dissolution of a domestic partnership to remove your domestic partner from your medical and dental coverage and stop or change your payroll deduction. Your ex-domestic partner will be sent notification about the opportunity to continue medical and/or dental coverages. While not required to provide COBRA continuation coverage, the university has elected to offer former domestic partners the opportunity to purchase continuation health and/or dental coverages for up to 18 months at full premium plus 2% administrative fee. COBRA rights will be forfeited if Benefits is not notified within 60 days of the termination of the partnership. Claims will not be paid for an ineligible ex-domestic partner beyond the end of the month in which the termination or dissolution is effective and premiums paid for the ineligible domestic partner coverage will not be refunded retroactively.

You also must provide, upon request, a copy of the Notice of Termination of Domestic Partnership filed with the California Secretary of State. Forms and instructions for terminating a domestic partnership are available from the State of California at www.sos.ca.gov/dpregistry. Dental coverage is not included in the medical plan; you will need to initiate a termination of benefits via eTrac to remove your dependent from your dental coverage.

 

Life Insurance and Accidental Death and Dismemberment Insurance

Dependent coverage for an ex-domestic partner ceases when the partnership is terminated. If the ex-domestic partner was the only eligible dependent you had covered, you will need to submit a Benefit Cancellation Form to terminate dependent coverage. You may wish to review your beneficiary designation(s) and update if necessary.

 

AFLAC

Contact AFLAC at (800) 992-3522 to discontinue your ex-domestic partner's premium from your payroll deductions. Your ex-domestic partner may continue coverage on a direct bill basis.

 

Long Term Care

Contact the carrier directly to discontinue the premium for your ex-domestic partner from your payroll deductions.

 

Retirement Plans

You may wish to review your beneficiary designation(s) and update if necessary.

 

Tuition Assistance

A registered domestic partner whose domestic partnership with a sponsoring employee has been terminated is not eligible to receive tuition assistance.
