Death of a Loved One

Medical and Dental Coverage

You must initiate an eTrac transaction within 30 days to remove your deceased dependent from your medical and dental coverage. You must also submit a copy of the Death Certificate. If you do not submit appropriate paperwork to remove your dependent you will continue to pay the premium for that dependent's medical and dental coverage until the end of the calendar year.

 

Flexible Spending Accounts

You may initiate or change the amount of a health care or dependent care FSA via eTrac within 30 days of death.

 

Dependent Life Insurance

If you have Dependent Life Insurance and the deceased dependent was the only eligible dependent you had covered, you will need to submit a Benefit Cancellation form to remove the dependent's coverage from your policies. You also will need to submit a claim for the Dependent Life Insurance benefit. If you have other dependents covered under your dependent life policy you will only need to submit a claim for the deceased dependent's life insurance benefit. Documents required for filing a claim include an original Death Certificate. It could take up to six months to receive the original Death Certificate. All documentation is then submitted by the Office of Benefits Administration to the insurance company for processing. You also may want to review your beneficiary designation(s) and update if necessary.

 

Accidental Death and Dismemberment Insurance

If you have family coverage under the Accidental Death and Dismemberment Insurance and the deceased dependent was the only dependent you had covered, you will need to submit a Benefit Cancellation form to remove the dependent's coverage from your policies. You also will need to submit a claim for the Accidental Death Insurance benefit if applicable. If you have other dependents covered under your Accidental Death and Dismemberment Insurance policy you will only need to submit a claim for the deceased's accidental death insurance benefit. Documents required for filing a claim include an original Death Certificate. It could take up to six months to receive the Death Certificate. You also will need to show proof of accidental death such as a police report. All documentation is then submitted by the Office of Benefits Administration to the insurance company for processing. You also may want to review your beneficiary designation(s) and update if necessary.

 

Long Term Care

Contact the carrier directly to discontinue the premium for your deceased dependent from your payroll deduction.

 

AFLAC

Contact AFLAC (800) 992-3522 directly to discontinue the premium for your deceased dependent from your payroll deduction.

 

Retirement Plans

You may wish to update your beneficiary designation(s) and update if necessary.
