Death of a USC Employee

Family members should contact the Office of Benefits Administration as soon as possible.

 

Medical and Dental Coverage

Medical coverage will terminate on the date of death. Surviving dependents will be sent notification about the opportunity to continue coverage under the provisions of COBRA. They will have 60 days to elect continuation under COBRA. If COBRA is elected, the surviving dependents may be covered under the continuation plan for up to 36 months by paying the full premium costs plus a 2% administrative fee.

 

Flexible Spending Accounts

Flexible spending accounts will be cancelled effective the date of death of the employee. Claims incurred prior and up to date of death may be submitted through March 31 of the following year.

 

Life Insurance

A family member must notify the Office of Benefits Administration as soon as possible in order to determine the beneficiary and to receive the appropriate paperwork to file a claim. Documents required for filing a claim include an original Death Certificate. It could take up to six months to receive the original Death Certificate. All documentation is then submitted by the Office of Benefits Administration to the insurance company for processing.

 

Accidental Death and Dismemberment Insurance

A family member must notify the Office of Benefits Administration as soon as possible in order to receive the appropriate paperwork to file a claim. Documents required for filing a claim include an original Death Certificate. It could take up to six months to receive the original Death Certificate. They must also show proof of accidental death such as a police report. All documentation is then submitted by the Office of Benefits Administration to the insurance company for processing.

 

Retirement Plans

A family member must notify the Office of Benefits Administration as soon as possible in order to be advised on the options available regarding retirement plan assets.

 

Tuition Assistance

Surviving children enrolled at the time of death of a current sponsoring employee may continue to receive 100% tuition assistance for a maximum of 144 undergraduate or 72 graduate or professional units in order to complete their degrees, subject to the terms and conditions of the university's tuition assistance benefit policy.

A surviving spouse enrolled at the time of death of a current sponsoring employee may continue to receive tuition assistance to complete the semester in which he or she is enrolled, subject to the terms and conditions of the university's tuition assistance benefit policy.
