Paid Family Leave Insurance Program

The Paid Family Leave Insurance Program is a state-mandated program that provides compensation for a period of up to six weeks for individuals who take time off of work to care for a seriously ill child, spouse, parent, or domestic partner (as defined by Section 297 of the California Family Code), or to bond with a new child due to birth, adoption, or foster care placement.
The disability of the seriously ill child, spouse, parent, or domestic partner must be medically certified and you must file a claim with the plan administrator to collect benefits. The benefit payments are limited to six weeks in any 12-month period and will not be paid if you are receiving disability, unemployment compensation or workers' compensation benefits.
Who's eligible?
Faculty and staff employees in California must participate in the Paid Family Leave Insurance Program.
Cost
The contribution for Paid Family Leave Insurance is included in the contribution to the USC Basic Disability Plan or State Disability Plan, which for 2008 is 0.8% of a taxable wage base of $86,698.

What do I need to do?
Eligible employees are automatically enrolled in the Paid Family Leave Insurance Program.
If you need to take time off work to care for a seriously ill child, spouse, parent, or domestic partner, or to bond with a new child due to birth, adoption, or foster care placement, you should first notify your supervisor and home department coordinator.
To file a claim, call VPA Inc. at (800) 495-2315 between the hours of 6:00 a.m. and 4:45 p.m. Pacific Standard Time. Your claim for benefits will be taken over the phone.
