The following forms are available to be downloaded to your printer. Once completed, please return them to the Office of Benefits Administration. Please note that additional documentation is often required to accompany some forms. If you do not have Adobe Acrobat on your computer, you may down-load it from the ITS Web site.
24 Hour Fitness
Staff and faculty can take advantage of low-cost corporate fitness rates to purchase memberships at 24-Hour Fitness centers throughout Southern California. Enroll on-line at www.24hourfitness.com/corp/UofSoCal using the discount code: 47608 or enroll in person at any of their fitness centers.
A representative is available at HSC in the Quad area (patio) from 2:00 p.m. to 3:00 p.m. on the first Wednesday each month.
