Local 11 employee medical plans

Medical and dental benefits are provided through the Los Angeles Hotel-Restaurant Employer-Union Welfare Fund.

Approximately 2 months after hire date, union will mail a packet containing explanatory material, an enrollment card, a medical plan choice card and a pre-addressed postage paid envelope to return the completed materials.

Employees may choose a Kaiser Permanente (HMO) plan, or the Indemnity Medical Plan (a fee-for-service plan in which employees choose their healthcare provider and will be reimbursed for part of their medical costs). If employee fails to choose one of these two plans, he or she will be automatically enrolled in the Indemnity plan. Employee can change their plan during open enrollment which takes place each May and is announced by mail.

Kaiser+ (Kaiser Permanente Deductible Plan) & Dental+ no charge
Kaiser2+ (Kaiser Permanente Traditional Plan) & Dental2+ $25 monthly employee contribution
Kaiser3+ (Kaiser Permanente Traditional Plan) & Dental2+ $50 monthly employee contribution
Indemnity3+ & Dental2+ $50 monthly employee contribution

Employees become eligible for benefits coverage in the month following 5 consecutive months of working 100 hours or more after the date of hire. For example, an employee hired in January would become eligible on July 1. Eligibility is maintained as long as employee works 100 or more hours in a month—if employee works less than that in any given month, eligibility ends at the end of the following month.
