I enrolled in the USC Network Medical Plan and have not yet received an identification card. When will I receive it?

You will receive your identification card approximately 15 to 30 working days after your eTrac transaction approval date. Your card will be mailed to your home address by RxAmerica, which is the pharmacy benefit manager. It will arrive in an envelope with "Member Card Enclosed" wording across the front of the envelope. Please note that all ID cards will carry only the name of the employee who enrolled in the plan as the subscriber. This card should be presented whenever you seek care from a physician, hospital, pharmacy or any other medical provider. If you don't receive your card within 30 days of eTrac transaction approval date, contact the Benefits office for assistance.

 

I am new to the Plan and want to know how to find a participating USC Network Medical Plan physician. What should I do?

For a USC Keck School of Medicine physician, go to the USC Network Medical Plan Web site at www.usc.edu/go/uscnetwork and click on "Search for Provider." For all other in-Network physicians, go to the Blue Cross of California Prudent Buyer Web site at www.bluecrossca.com or for participating BlueCard program providers go to www.bluecares.com. If you can't find a suitable physician there, contact the USC Health Plans office for assistance.

 

I want to make an appointment with a physician I have chosen from the Network/Blue Cross Web site. What should I do?

Call the doctor's office and set up an appointment. If you are seeing a USC Keck School of Medicine physician, tell the office you are covered by the USC Network Medical Plan. If you are seeing a Blue Cross Prudent Buyer physician, tell the doctor's office that you are covered by the USC Network Medical Plan and the Plan uses the Blue Cross Prudent Buyer Network. If you need to access a provider outside of California but within the U.S., identify yourself as having the Blue Cross/Blue Shield BlueCard Plan.

 

I want to see a specialist, what kind of referral or authorization do I need?

The USC Network Medical Plan does not require a referral or authorization for a specialist visit. It is important, however, that you tell any doctor you see about any other doctor visits you may have had recently and about all medications you are taking. The HMOs do require a referral or authorization for certain services.

 

I received a billing statement from a health provider even though I have the USC Network Medical Plan. What should I do?

Delta Health Systems processes claims for the USC Network Medical Plan. Contact Delta Health Systems for the payment status of your claim or go to their Web site at www.deltahealthsystems.com and click on e-Health. There you can view your claims history and determine if your claim has been received and recently processed. If processed, you will be able to view your patient responsibility. If you are unsuccessful in resolving a claim issue with Delta Health Systems, contact the USC Health Plans office for assistance.

 

How do I receive services under the Vision Service Plan?

The Vision Service Plan does not issue an identification card. However, your ID number is the same number as your subscriber ID number located on your USC Network Medical Plan ID card. You can call an optometrist directly to make an appointment. In order to receive the highest benefit you must verify that the optometrist is a current VSP provider. To check your eligibility, view your personal benefits and/or to locate a participating VSP provider, visit their Web site at www.vsp.com or telephone VSP at (800) 877-7195.

 

I am enrolled in Plan C; how do I obtain reimbursement for my prescription co-pays?

You can download a claim form from the USC Benefits Administration Web site at www.usc.edu/benefits/forms.html. Complete the Network medical claim form, attach your original prescription receipts and mail both to Delta Health Systems, P.O. Box 780, Stockton, California 95201-0780. You should always send Delta the original receipts and retain copies for your files.
