Required Arrival Form
If you have applied to live in Academy housing or you would like to have shuttle service from LAX to USC when you arrive for Academy courses, you will need to download the Required Arrival Form and fax, email or mail it to our offices. This form must be received by the deadline in order to secure your housing space and guarantee shuttle service.
 Spring 2010 Required Arrival Form
After we receive this form, we will contact you to confirm it was received and we will then send your shuttle instructions and reservation number to the e-mail provided on the form.
