The following is a list of forms that you will need to apply for either the online Graduate Certificate Program or the online Master's of Arts in Gerontology (MAG) Program:

Applicant's Checklist

1. Completed USC Graduate Application (including the USC Supplemental Financial Aid Form)
   
2. School of Gerontology Supplemental Application
   
3. Current Resume
   
4. Graduate Record Examination scores.
   
5. TWO sealed transcripts from all undergraduate and graduate institutions attended.
   
6. Two letters of Recommendations
   
7. (International students only) TOEFL score report
   
8. Application fee of $55.00
