Seniors Fit for Life

At USC, we recognize that healthy aging requires the support of a multidisciplinary team experienced in geriatric medicine. With this in mind, the USC Division of Geriatric and General Internal Medicine established the USC Seniors Fit for Life Program. This monthly, half-day consultation program provides seniors with the opportunity to meet an integrated team of geriatric specialists who come together to assess their specific health care needs and ensure that they receive the latest and most appropriate therapies.

Each of our specialists is experienced in the many health and social issues that challenge seniors today. Seniors meet individually with a USC board-certified geriatrician, pharmacist, dietician, occupational therapist, physical therapist, and social worker during a four-hour period. The team addresses important senior-specific issues such as Alzheimer’s disease, dementia, fall prevention, medication review for drug interactions and cost, healthy eating, end-of-life planning, and access to home care. As a team, these specialists work together to coordinate health care that addresses individual needs.

Home Visit Program

As advances in medical technology help seniors live longer, the geriatric specialists at USC are finding new ways to help seniors maintain quality of life. Central to quality of life is the ability to live in one’s own home, rather than in a long term care facility such as a nursing home. However, for the many seniors unable to venture away from their homes because of declining physical condition, vital health care is increasingly difficult to obtain.

Through USC’s innovative Home Visit Program, our geriatric specialists bring crucial medical care to homebound seniors, who find visiting a physician’s clinic a strenuous physical challenge. At the patient’s request, a geriatrician and a geriatric nurse specialist will visit the patient’s home, manage medical issues, call-in pharmacy prescriptions, obtain blood samples for testing, coordinate care with a home health agency, assess home safety, help obtain in-home supportive services, and much more. This team of specialists is supported by a full staff of dedicated medical professionals. At USC, we understand the importance of delivering quality medical care where it is needed most.
