In Case of a Work-Related Injury at CCMB

If an employee is injured on the Health Sciences Campus, medical care will be provided at the following locations:

During regular hours (8:30AM - 5:00 PM Mon-Fri):

If an ambulance is required, contact the HSC Department of Public Safety at (323) 442-1000. They will contact paramedics and send personnel to the injured worker until the paramedics arrive to take over treatment and transportation.

The employee may also elect to visit a designated personal physician. The employee must previously have submitted a form available from the CAPS Website: http://capsnet.usc.edu/WC/webPDF/PhysicanForm.pdf

When someone is injured, they must immediately report this to their Supervisor who fills out a form entitled "Supervisor's Report of Injury". If possible, this form should accompany the injured employee to his/her first doctor's visit. This form is available at: http://capsnet.usc.edu/WC/webPDF/SupervisorRptOfInjury.pdf
The Employee must also fill out a form entitled "Employee's Claim for Workers Compensation". This form is also is available at the Worker's Compensation Website at: http://capsnet.usc.edu/WC/webPDF/ClaimForm.pdf

State law requires that all (major and minor) injuries be reported within twenty- four (24) hours. For Injuries that occur over the weekend or after regular business hours (Monday thru Friday, 8:30 AM to 5:00 PM), reports should be completed and sent to the Workers Compensation office the next regular working day after the accident. Please call us to let them know about the injury and that the report is on the way.
