The School of Dentistry's Office of Quality Assurance now requires all volunteers at the School of Dentistry and extramural clinics to submit a USCSD volunteer agreement form prior to involvement in volunteer activities.

The agreement is required for both the protection of the individual and the school and permits volunteers to be covered under the school's malpractice and general liability insurance.  The agreement must include the volunteer's specific assignment details, time frame of duties and supervisor’s signature.  Each volunteer's identity must be confirmed with picture identification.

The Office of Finance, Planning and Human Resources, the clinic or area where the volunteer activities will take place, and the volunteer must be given a copy of the volunteer agreement.

No volunteer may engage in direct patient care or sterilization activities under any circumstances.
