FACT SHEET INSTITUTED IN
CLINICS
In accordance with guidelines established by the Dental Board of California, all dental patients must receive information regarding the restorative materials that may be used during the course of their treatment. The Dental Materials Fact Sheet is mandatory in all clinics rendering restorative care and must be reviewed with all patients prior to initiating treatment. Patients must sign the Patient Acknowledgment of Receipt of Dental Materials Fact Sheet upon completion of review, and the form must be placed in the therapy notes section of the patient’s chart. In addition, patients who are presently being treated must be given the fact sheet and sign the acknowledgement receipt at their next visit. New patients receive this information during the admitting process.
The advanced periodontic, orthodontic, oral surgery and dental hygiene clinics are not required to complete the fact sheet.
The fact sheets and acknowledgement forms are available in all group practice offices, the patient admitting office and information desks on the first and second floors.
