The form below is to be filled out by USC faculty and staff needing to update the registration for a host that is already registered for use on the campus network (USCNet). New computers on USCNet must have their ethernet hardware address registered even if the computer will be using a host name and IP address of a currently registered computer.

To update the information you must provide either the host name or IP address to be updated and the new ethernet hardware address. If you want to obtain a new IP address visit ITS's IP Request site.

This form can also be used to obtain a new IP address if you are moving the computer to a different building. Please indicate that information in the comments area.

Ethernet address changes can also be done online via the pages used to lookup existing host information.
