Service Level Agreement Forms


A signed Service Level Agreement (SLA) is required prior to the use of any ITS Multimedia services, support or equipment. The SLA may exist at the level of the school, department, or individual. Faculty members should consult with their department media coordinator to see whether there is a standing SLA in place for their departments or schools.

The Service Level Agreement defines the ITS Multimedia Service and Support offering and enumerates both ITS Multimedia and USC faculty responsibilities related to the use and availability of audio-visual equipment in specified auditoria and classrooms.

Authorized Users List
Department Multimedia Coordinators: Use this Microsoft Excel template to record faculty, TAs, guest instructors, and speakers who are authorized to use multimedia equipment in general-use rooms that are supported by ITS Multimedia.

SLAs that are on file with ITS are auto-renewable every July 1. A school or department that has already submitted a signed and approved SLA will not need to take any further action. An existing SLA may be changed or cancelled by sending a request to: multimedia-L@usc.edu. An example of a change requiring notice to be given to ITS, would be a personnel change of a department authorized signer (SBO).
