Communications Services Forms Overview

This section provides forms that you will need to fill out and submit to request communication services, including telephone and data network services, voicemail services, and a range of telephone services.

Completed forms may be mailed to:

Forms may also be faxed to 213-740-7254.

In addition, Communications Service Request forms for voice, data network, and/or repair service and Telephone Set Maintenance forms may also be submitted online.

Introducing TMIS

In summer 2013, ITS will replace the Telecommunications Management System (TMS) with the new Technology Management Information System (TMIS) to integrate and streamline the ordering of voice and data services at USC. For additional information on TMIS, see www.usc.edu/its/telecom/tmis.html
