Telephone and Network Services
Welcome to the ITS Telephone and Network Services Website. This page will answer key questions and help you to locate necessary forms.
Completed forms may be delivered to the Communications Services Group (CSG), CAL 319, MC 2812, or faxed to 213-740-7254. Communications Service Request and Telephone Set Maintenance forms may also be submitted online. Please see the side menu on the right side of this page.

Overview of Telephone and Network Services
The Technology Connection Charge, or TCC, covers the cost of basic telephone and data network services for USC schools and departments.
Basic telephone services include dial tone, voicemail, 5-digit (extension) dialing, and local and long-distance calls within the U.S. The TCC also provides troubleshooting to the phone jack and maintenance of inside wiring. Units may choose the ITS telephone maintenance plan for their digital telephone sets or may maintain their own telephone sets. Units may also choose whether to purchase new digital telephone sets from ITS or from a third-party vendor.
Basic data services include wired, wireless, and dialup access. The TCC provides data network maintenance up to the building. Units may choose to have ITS support their internal (intra-building) data network for an additional 12% of their total TCC.

Communications Services Forms
Voice, Data, and Repair Forms
These forms are used for installing, moving, disconnecting, and repairing voice and data services.
For voice services, use the Communication Service Request (CSR) form to order telephones, calling cards, mobile phones, pagers, and point- to-point circuits. You may submit your request through the online CSR or use the downloadable Word version of the CSR.
For data services, use the Activate, Deactivate, or Move Data Service form.
For repairs of voice or data services, use the Report a Phone or Ethernet Problem form or call the Help Desk at 213-740-4357.
Voicemail Service Request Form
The Voicemail Service Request form is used to add, delete, and change voicemail options on an existing phone line. If you are requesting voice mail for a new telephone number, you should complete both the CSR form and the Voicemail Service Request form.

The Long-Distance Caller Identification Number (CID) allows you to make long-distance phone calls from university telephones that have a restricted calling range, such as phones in reception areas or hallways. Use the form below to create, change, or delete a CID account.

Telephone Set Maintenance Form
The ITS telephone maintenance plan is available to individual faculty and staff members, as well as to entire departments or schools.
You may submit your request with the online Telephone Set Maintenance form or use the downloadable Word version of the Telephone Set Maintenance form.
