Telephone Set Maintenance Request

Opt In

The individual or department chooses to have ITS maintain the indicated digital/ISDN telephone sets. ITS maintenance support includes any necessary repair or replacement of a digital/ISDN telephone set. Support services are available from 8:30 a.m. to 5:00 p.m., Monday through Friday; weekends and university holidays are excluded. Analog sets are not included or supported under this option. Departments will be billed an annual maintenance fee of $48.00 for a 20-button set or $60.00 for a 50-button set.

All maintenance contracts run through June 30. Contracts beginning January 1 or later are prorated at half the annual rate.

If you are an SBO and want to opt in for your entire school or department, please list your school or department name in the box below. You will be contacted by ITS to confirm enrollment.

Opt Out

The individual or department chooses to opt out of the ITS telephone set maintenance plan and assumes responsibility for all analog and digital/ISDN telephone set repair and replacement.

In the event of a customer-initiated trouble report, the department will be billed a flat-rate charge of $100.00 based on the average cost of a service call. If the problem is created by the telephone set, the department will be responsible for replacing the telephone set. Departments may purchase replacement sets from ITS by filling out Form C1.

Individuals or departments that opt out of ITS telephone set maintenance and choose to maintain their own telephone sets must:

  • Ensure that the telephone set is compatible with the current university telephone service.
  • Ensure that the telephone set adheres to ITS requirements.

ITS requires six-month's notice for service changes. Checking the Opt Out box prior to January 1 will result in the phone being removed from ITS handset maintenance on July 1. For more information, please visit our website at www.usc.edu/its/pricing/basic_fees/telephone_support.html.
