Voice, Data, and Repair Forms
These forms are used for installing, moving, disconnecting, and repairing voice and data network services.
For voice services, use the Communication Service Request (CSR) form to order telephones, calling cards, mobile phones, pagers, and point- to-point circuits. You may submit your request through the online CSR or use the downloadable Word version of the CSR.
For data network services, use the Network Jack Activation, Deactivation, or Move form.
For repairs of voice or data network services, use the Report a Phone or Ethernet Problem form or call the ITS Communications Services Group (CSG) Help Desk at 213-740-4357.
