Forms for First-Time Visitors
If you are accessing CWFL services for the first time, or you are returning to CWFL with a new concern and it has been more than six months since your last visit to us, you will be asked to complete the forms below.
These forms are available in both of our offices, for you to complete in person. If you use this option, please plan to arrive ten minutes before your scheduled appointment, in order to complete the forms and maximize your session time with your consultant.
Another option is for you to download and print these forms, so that you may complete them at a time of your convenience, and bring them in with you.
Because your answers to these forms may contain sensitive or private information, we encourage you to keep the forms securely in your possession until you arrive. We do not encourage you to fax or email these to us, as these options may result in errors that deliver your materials to an unknown party.
