Verification of Completion of the Requirements for the Doctoral Degree

This form is designed to help ensure that all your degree requirements have been met.

  1. Fill in your name, address and USC ID number.
  2. Attach a copy of your STARS report, and submit the form to the Graduate Advisor at the time of your dissertation defense.
  3. The departmental Graduate Advisor will review the STARS report and, by signing the form, verify that all degree requirements have been met.
  4. The Dean, Department Chair or Director will then sign the form.
  5. The Graduate Advisor will submit the form to The Graduate School.
