Forms for Doctoral Qualifying Exams and Defenses

Doctoral Students

• At least 2 semesters before the qualifying examination, the Appointment of Qualifying Examination Committee form should be completed.




• At least 30 days before the start of the qualifying examination, the Request To Take the Qualifying Examination should be completed.




• As soon as possible after passing the qualifying examination, the Appointment of Dissertation Committee should be completed.

After downloading the appropriate form, obtain the signatures of the committee members and then submit the form to the department chair. The form will be sent to the dean for review and approval, and a copy will remain in the department.

If changes need to be made to the Qualifying Examination or Dissertation Committee, a Change of Committee form should be completed and submitted to the department chair as soon as possible.
