Submission Packets
You can download and print all documents required to submit your thesis or dissertation. Make sure you are aware of all submission deadlines for your graduation paperwork and for your manuscript. You are also strongly advised to ask your department or school about any additional forms and procedures.

- Approval to Submit Defended and Final Copy of Doctoral Work form
- Verification of Completion of the Requirements of the Doctoral Degree form
- Survey of Earned Doctorates
- Approval to Submit Final Copy of Master's Thesis form
- Verification of Completion of the Requirements for the Master's Degree form
