Checklist for Applicants to the Department of Anesthesiology at USC

You will need to provide the following information when applying for a residency position to ensure we have a complete application:

• Complete application through ERAS
• Passport size photo
• Curriculum Vitae.
• Personal Statement.
• Medical School Transcripts.
• Dean's Report.
• ECFMG if applicable.
• USMLE 1 and 2 scores (or equivalent if you are a DO). If you are currently a 4th year medical student, USMLE 2 is not a requirement.
• If applicable - "Applicant Evaluation Status Letter" from the Medical Board of California stating you are eligible to commence postgraduate training in California.
• Copy of diploma and translation if applicable.
• Visa status as applicable.
• At least 3 recommendation letters and if possible one from an Anesthesiologist.
• California License and DEA if you have these.
• Certificates or letters of completion of Internship and/or residency done in the United States.
