Central Application Service for Physician Assistants

Only candidates who have completed their CASPA file and have submitted their USC PA Program Supplemental Application by December 1, 2008 will be reviewed for interview consideration.

Additionally, official scores of the General and Analytical sections of the Graduate Record Examination (GRE) must be submitted to USC by using the Institutional Code 4852(departmental code is not necessary) no later than December 1, 2008. Applicants with MCAT scores must provide their AAMC verification code to the USC PA Program.

Only candidates who have taken their examination by December 1, 2008 will be reviewed for interview consideration. The examination must have been taken within five (5) years of the coursework deadline of June 30, 2009.

Information about CASPA can be obtained by visiting https://portal.caspaonline.org/ or through emailing CASPA at caspainfo@caspaonline.org or by calling (617) 612-2080.

Please note: Although the deadline to submit the completed CASPA application, including all supporting documents is December 1, 2008; the PA program highly recommends submitting the CASPA application by October 1, 2008.

USC PA Program Supplemental Application

Applicants are required to submit the USC PA Program Supplemental Application.

Only candidates who have submitted their Supplemental Application will be reviewed for interview consideration.

The Supplemental Application can be obtained by clicking on the "Application Form" link on this website or by clicking on the PDF link below. Complete and submit the Supplemental Application, all supporting documents and the $50 processing fee (please make all checks payable to the USC PA Program) by October 1, 2008 and no later than December 1, 2008 directly to:

USC Physician Assistant Program

Note:
The processing fee can be waived under certain circumstances. Please contact the PA Admission Office for more information.

The following supporting documents are required:

  • Copies of certificates, and current licenses to provide evidence of civilian/military medical training (if applicable)
  • Copies of Form DD-214 to provide evidence of completion of military training (if applicable)
  • If coursework is not already posted by CASPA, then unofficial transcripts reflecting all undergraduate coursework.

Note:
Initially, please complete only the CASPA report and the Supplemental Application. The USC PA Program does utilize the USC Online Graduate School Application. However, only admitted candidates need to complete that application, which includes an $85 fee. Instructions will be given at the time of admission.

The USC PA Program Supplemental Application must be filled out completely, with questions marked “not applicable” where appropriate. Attach additional pages as needed to complete answers in any particular category. Also, remember to sign and date the form. Do not alter the application in any way. The form and all supporting documentation become the property of the USC PA Program and neither the original form and supporting documents nor photocopies of them will be released or returned.

  • Important:
Re-applicants must submit new materials to CASPA and the USC PA Program during each new application year.

International Students

In general, prospective applicants who have completed their education in countries other than the United States must secure a report by an international transcript evaluation service to preliminary verify two completions: 1) equivalence of a bachelor's degree; and 2) admission prerequisites. Two semesters of college-level English composition also are required. In addition, the USC PA Program may require applicants to take the TOEFL to indicate proficiency in English. If granted an interview and offered admission to the USC PA Program, applicants must submit official international transcripts to USC for verification of the preliminary, unofficial review conducted by the evaluation service.

In addition to the above, complete and submit the USC 2009-2010 Online Graduate School Application and all supporting documents no later than December 1, 2008.

The USC 2009-2010 Online Graduate School Application is available at: http://www.usc.edu/admission/graduate/applyonline/

Apply for financial Aid

General information and application forms for financial aid and financing a USC education are available at: http://www.usc.edu/admission/graduate/fa

Note:
It is always a good idea to (i) apply early for financial aid; (ii) keep photo copies of all papers submitted; and (iii) follow-up on status of financial aid application to ensure that all documents were received in time for review.
