Application for Fellowship Training in Neuroradiology

A completed application includes:

1. Your updated curriculum vitae (please include your social security number
and indicate your present service and teaching responsibilities).

2. Three letters of recommendation from those who have trained you in
Radiology, including your present Chief.

3. A completed application form; include the date on which you could
commence Fellowship training.

IT IS ESSENTIAL FOR OUR FELLOWS TO BE IN POSSESSION OF A CALIFORNIA LICENSE PRIOR TO COMMENCING THEIR TRAINING.
