Please use email, attaching a spreadsheet, or any convenient means at your disposal, to send new Postdoctoral Research Scholars and Visiting Research Scholars contact information who come on board at KSOM throughout the year for the following titles and job codes listed below:
- Visiting Research Scholar

- Postdoctorate Fellows (098203, 098207, 098211, 098215)
- Research Associate (098063)

- Fellowship Holder (032020)
Procedures for New or Re-Appointed Postdoctoral Scholars: (Postdoctoral Research Associates, Postdoctoral Fellows and Fellowship Holders)
- Please email new data to revangel@usc.edu
- Mail a hard copy of the appointment letter to Becky Evangelista, KAM 506/MC9023.
- Mail a hard copy of the demographics page for Postdoctoral Fellows only (job code 098203) to the same address as above.
Health Benefits for Postdoctoral Fellows and Fellowship Holders (Job Codes 098203, 032020) Each Term (Instructions for these two job codes only)
If both appointment letter and demographics page is on file in the Office of Research Advancement then you need only send an email request with the postdocs name(s) and the term for registration. The office will notify you if more information is needed. For the financial component, please send the following items to Becky Evangelista, KAM 506/MC9023:
- Internal Requisition
- Memo stating what the requisition covers, the postdoc's name and 10-digit USC ID number
- The Budget Page for the account you will use to pay for the health benefit fees
In order to register postdoc fellows and fellowship holders for health benefits the Office of Research Advancement will submit the Appointment/Offer Letter along with the Demographics Page to the Registrar for them to be enrolled in the PDF 999 class. Once registration is confirmed then a spreadsheet of the postdocs for that term will be sent to Health Services with a request for graduate student health benefits.
Required by either Health Services and/or the Registrar are the following:
- Appointment Letter
- First name
- Last name
- Social Security #
- 10 digit USC ID # (unless Postdoc is new, please send once they have an ID number)
- Date of Birth
- Home Address
Either the Department, the P.I., or the Fellowship pays for the postdoc's health benefits. This is arranged on the departmental level between the P.I. and the Dept.

If the postdoc wants to put his family on his USC graduate health service benefit then once the insurance is active he/she will need to contact Blue Cross at 800-888-2108 to enroll the family.
The postdoc would be paying out of pocket for this Dependent Plan.
If the Postdoc has insurance external to what USC offers then the department needs to verify this. Insurance, whether USC based or external is a requirement of USC.

Scholars need to alert their departmental home coordinators to process visa renewal requests before they expire. The home department does this through Immigration Personnel Services Fellowship Officer, Cecilia Melendres. Please make sure your scholars are aware of this.
