Step Two: Complete the Student Recital Request Form

This form is available in the Music Operations Office (BMH-101). After completing the form, non-doctoral students please turn into Music Student Affairs (LPB 200) for final approval. Doctoral students must submit their form to Dr. Debora Huffman (MUS 402) for approval. They will submit the form on your behalf to be included in the next lottery.
