Anticoagulation Management

Anticoagulants, such as warfarin, are given to help prevent abnormal blood clotting (thrombosis) in blood vessels. Generally, anticoagulation therapy is used to treat patients with deep-vein thrombosis (DVT), pulmonary embolism, atrial fibrillation, or prosthetic heart valves. Close monitoring of patients taking warfarin is important since the medication can be affected by other medications, vitamin K- enriched foods, herbals or over-the-counter products. Clinical pharmacists are available to help patients manage their anticoagulation therapy.
