USC School of Pharmacy Supplemental Application

Applicants who apply to the University of Southern California School of Pharmacy must submit a USC Supplemental Application in addition to their PharmCAS application.

USC Supplemental Application for Fall 2010 is a two-part process: Part One, the USC Graduate Admission Application, and Part Two, the Pharm.D. Supplemental Application. The Part Two Doctor of Pharmacy (Pharm.D.) Supplemental Application is accessible once you submit Part One, the Graduate Admission Application.

Preparation for the Supplemental Application
Below are the necessary steps you need to take to prepare to fill out your USC School of Pharmacy Supplemental Application. You may want to print out this page for reference before you start your application.

  • Visit the USC School of Pharmacy Web site to review our:
    Pre-Pharmacy Requirements and Course Equivalencies for equivalent courses at your school(s).
  • Start the Part One application here by creating an account first. Part Two (Doctor of Pharmacy Supplemental) application becomes accessible once you submit your Part One (Graduate Admission) application.
  • You must fill out each section of the application denoted by an asterisk, which indicates required information. You may skip the sections not required by Pharm.D. program, such as GRE, TOEFL exams, etc. Please note that the Pharm.D. program is a full-time program. The "Distance Education" option is not available for this program. A Statement of Purpose under Part One of the application is NOT required for Pharm.D. application. Instead, you will be asked to submit your answers to the Application Issues in Part Two.
  • Have transcripts available from each higher education institution that you attended from which you will select the courses used to satisfy the pre-pharmacy requirements. You may use PharmCAS rules to calculate your cumulative undergraduate GPA.
  • Arrange for two letters of recommendation submitted on-line through PharmCAS. One letter should be from a faculty member and a second from a person who can best describe your capabilities and fitness for a pharmacy education and career. Re-applicants only need to send in one new letter of recommendation. Please ignore the "Recommendation" link on the left hand side of the Part One application.
  • Submit your resumé through the USC Supplemental Application online under "Employment Information/Resumé."
  • Any format is acceptable.
  • You must respond to two of three Application Issues in Part Two Doctor of Pharmacy (Pharm.D.) Supplemental Application. You may write your responses in a separate Word file and "cut and paste" it onto the online application form. Please limit your answer to each question to no more than 300 words, or as long as it is allowed on the application.
  • You must sign electronically the California State Board of Pharmacy Felony and Misdemeanor Conviction Disclosure Statement.
  • You will be required to submit electronically a (non-refundable) Supplemental Application Fee of $85.00. If you select the fee waiver option due to financial difficulty, you will be asked to provide supporting documents. This may delay the evaluation of your application.
  • You must mail the following items directly to the USC School of Pharmacy Office of Admission and Student Affairs:
    (Do not send the transcripts to the USC Graduate School)
    1. One official transcript from each higher education institution attended, including summer and extension sessions. All transcripts must be in sealed and taped envelopes and marked with school seals. No cover page is required. Envelopes that appear to have been opened will be returned. Further, you must submit updated transcripts at the close of each subsequent term, as soon as they are available. The School of Pharmacy will forward a copy of your transcripts to the Graduate School after evaluation. Re-applicants only need to send in new/updated transcripts/grades that you did not send us from the last application.

      All applicants with international coursework must arrange for delivery from the issuing institution directly to the University of Southern California School of Pharmacy’s Admission Office one copy of a certified transcript, comprehensive course descriptions and syllabi which will be used by appropriate university personnel to evaluate international coursework. Certified English translations of international transcripts must also be submitted to the University of Southern California School of Pharmacy.

    2. Proof of Permanent Residency (Alien Registration)
      If you were not born in the US but hold an Alien Registration Card, you must provide two (2) photocopies of the front and back of your registration card.

      If you are not a U.S. citizen nor do you hold an Alien Registration card, and an international student (with an F1/F2, H or other visa), you must provide documentation of your immigration status.
