Waiver Criteria

Students who are enrolled in an alternate health insurance plan (or who are on their parents' plan) may waive (relinquish) enrollment in the USC Student Health Insurance Plan by providing proof of comparable coverage (see criteria below). All insurance waivers, for the Fall Semester must be submitted before September 12, 2008.

Criteria for Waiver

To waive the USC Student Health Insurance Plan, your other coverage must:

Must provide at least $500,000 in lifetime aggregate coverage (no per incident maximum, no major exclusions)

Must cover the student for the entire coverage period

Must have effective dates covering entire semester:

-  Fall Semester 2008 – August 18, 2008 to January 11, 2009
-  Spring/Summer Semesters 2009 – January 12, 2009 to August 16, 2009

Must pay for at least 80% of covered expenses (not just for emergency care) in the Los Angeles area

Must provide verifiable proof with the student’s name (ID card, insurance policy or letter from your insurance company)

Must have a deductible of $2,500 or less, per policy year, in the Los Angeles area (lower deductible may be advisable)

The plan description must be provided in the English language, currency must be converted to U.S. dollars and there must be an insurance contact telephone number in the United States

Waiver Deadlines

The waiver request must be submitted by the following dates:

Fall 2008 waiver deadline – September 12, 2008

Spring/Summer 2009 waiver deadline – January 30, 2009

 
IMPORTANT

Students insured under their parent’s plan, can face the possibility of not being covered in the Los Angeles area if their managed care plan does not provide local coverage. It is best to first check with your carrier if you are waiving the USC insurance plan.
