Waiver Form

The university requires that ALL students carry supplemental health insurance. All students carrying 6 units or more are automatically enrolled in, and charged for, the USC Student Health Insurance Plan. All International and Health Sciences Campus students are automatically enrolled in the health plan even if carrying less than 6 units.

Enrollment will only be waived if documented proof of coverage from another health insurance plan, that meets the University’s criteria, is presented to the Student Health Insurance Office.

Coverage is for the Spring/Summer Semester only from January 1, 2010 through August 15, 2010. The Waiver Deadline is January 29, 2010.
