  Fees: Your health fee covers most medical services at the UPHC, however there are nominal fees for photocopying medical records. If you are not a student, please call the Health Information Management for photocopying charges.
Students: first page $1.00
additional pages $0.50 each, maximum $10.00

 

About this service

Your medical records are a confidential record that is created when you visit any UPHC medical services provider, or Health Promotion and Prevention Services staff member for an individual consultation. If you visit the Counseling Services, you will also have a medical record on file in that location.

No one is authorized to see your medical record without your written authorization if you are over 18 years of age, according to the Confidentiality of Medical Information Act (CMIA). Exceptions to this include: medical emergencies, utilization review, quality assurance activities, court order or subpoena, and licensing and accreditation of the facility. For more information see California Civil Code 56.10.

How to utilize this service

You are entitled to see and have copies of your medical record. You must present a photo ID in order to review or have copies of your medical record.

To review your medical record, call the Health Information Management office 24 hours in advance.

To obtain a copy of your medical record, you must complete and sign an Authorization Form for Release of Medical Records. It will take 5 days to process your request.
