	Hours:	Travel consultations are available on Wednesdays only, during general medical services hours

	Fees:	Travel consultations are included in your health fee. During the summer, non-matriculating USC students may receive a consultation for $25. There are nominal fees for physical examinations, prescription medications, immunizations and laboratory tests. See the fee booklet for a complete listing of fees.

If you are planning to travel abroad, we offer personalized travel consultations through UPHC medical services.

Through the Travel Clinic you are able to receive a personal consultation tailored to your own travel itinerary! Your consultation includes information on:

How to utilize this service

Consultations are by appointment only. Please make appointments well in advance. Some immunization series are most effective if started six months prior to the date of travel. We recommend that you allow yourself at least 8 weeks to adequately prepare for travel.

If you would like to schedule a travel consultation, please fill out a Travel History Form and submit it to the UPHC Immunization clinic. You may also fax it to us at: (213) 821-2740. Once you have submitted this form, please call (213) 740-0472 to schedule an appointment.
